By using the Families for Depression Awareness Mental Health Family Tree tool, you agree to these terms and conditions of use.
Purpose and Limitations
The Mental Health Family Tree tool ("the tool") is made available for your personal use, sharing with your family, and discussion with your healthcare providers. When completed, your Mental Health Family Tree may indicate patterns of behaviors and mental health diagnoses that can be helpful to healthcare professional(s) as they conduct an evaluation of your or your loved one's mental health.
Please note that the tool- does not diagnose depression, bipolar disorder, or any other mental health condition
- is not intended to, nor does it, provide medical or mental health care
- is not a substitute for professional medical and/or mental health care
- is intended for people age 18 and older
Role of Healthcare Providers
If you or a family member experience symptoms of any mental health condition, or if you have questions or concerns, you should consult a licensed medical or mental health care professional ("healthcare provider"). If you are under age 18, you should talk to your parent(s) or guardian about seeing a relevant healthcare provider.
Limitation of Liability
Families for Depression Awareness and its staff, consultants, and funders shall have no liability for claims by, or damages of any kind whatsoever to, a user of this Mental Health Family Tree tool, nor to any other person for a decision or action taken in reliance on the information provided in the Mental Health Family Tree tool. Such damages include, but are not limited to, direct, indirect, special, incidental, or consequential damages. By building your Mental Health Family Tree, you are agreeing to these Terms and Conditions of Use.
