Coming together with a plan

Fortunately, there are many strategies you can use to create a plan that will help you support your loved one. One such strategy is to watch our video, which outlines five manageable steps you and your family can take to develop a plan.

In addition to the video, you can download the “Making An Action Plan” template, which will guide you through the process and provide a framework for your discussions.
